Liz’s compassion and non-judgmental relationship style will help you relax into the work so that you maximize your therapeutic journey and actually look forward to connecting. Her confidence will empower you to unlock the inner strength and resilience that lives inside of you.
You will know you are in good hands with Liz in your corner, whether it’s her specialty in working with trauma, adoptees, or struggles with mood challenges like anxiety or depression.   Her experience and training span many areas of mental health, which means you will feel well matched no matter the turns that your journey may take. You can show up as you are and become equipped with tools to grow meaningfully from there.

Liz’s areas of expertise include, but are not limited to, EMDR, Cognitive Behavioral Therapy (CBT), mindfulness practices, and Internal Family Systems (IFS).

CLINICAL EXPERIENCE

Before joining The Viva Center, Elizabeth held numerous clinical leadership positions at a variety of local, regional, and national mental health organizations. In her almost 20 years of clinical work, she revels in the opportunity to affect positive wellness outcomes through her supervisory influence as well as her direct clinical work. 

EDUCATION

Elizabeth received her Master of Science in Advanced Clinical Social Work from Columbia University, New York, New York, and her Bachelor’s degree in Human Development and Family Science from Ohio State University in Columbus, Ohio.

She has proven her continued dedication to her clients and their wellness throughout her academic and professional career.

For the first visit, we will ask you to fill out the following forms.

Waiver of Liability & Informed Consent

Each of our Clinical Associates is a licensed practitioner. As required by licensure, treatment under a licensed practitioner is confidential. It also outlines the collaborative nature of the therapeutic relationship and co-responsibility for treatment outcomes.

Telemental Health Informed Consent to Treatment

Outlines components, limitations, risks, and benefits of telehealth treatment. Including what to do in the event of an emergency.

Financial Agreement

This form outlines The Viva Center’s financial policies, rates, and out-of-network status.

Client Questionnaire

This form serves 2 purposes: It helps to capture the baseline experience when beginning treatment, allowing for an opportunity to take stock of current status. It also helps the clinician gather important background information without taking extra time in the first session. The first page of this form is required, and the others are supplemental. It is fine to exercise the decision to leave out any areas of the form.  Please keep in mind any areas elected not to be disclosed at this time may be relevant to treatment outcomes.

Notice of Privacy Practices

Describes our commitment to keeping your protected health information secure. Details how it can and cannot be used or disclosed.

Exceptions to Confidentiality

This form explains limitations to the confidentiality required by licensure.
