Faith Ferber is dedicated to affirming and supporting you – as an individual, couple, or polycule. Faith will equip you with concrete tools for wellness that transcend the therapy room and can be applied across different facets of your life. Her thorough expertise about the unconscious processes that determine mood, behavior, and felt states in the body is an asset to your most successful therapeutic experience as well as all the joy that comes from effective wellness results. As a self-described “forever student”, Faith offers you a thorough understanding of both brain and body based approaches that can expedite you reaching your therapeutic goals. Her unwavering belief in each of her clients inner wisdom as the most critical therapeutic asset means your therapeutic journey will be uniquely guided by her affirming and supportive relationship style.
Faith has had a wide array of therapeutic training and can incorporate and shift between modalities as it meets your needs. Her modalities and areas of expertise include Brainspotting, psychedelic-assisted therapy, somatic techniques, solution-focused therapy, cognitive behavioral therapy (CBT), strengths-based therapy, motivational interviewing, and mindfulness.
I believe everyone has the inherent right to a safe and fulfilling life free from the harms of violence and oppression.

For the first visit, we will ask you to fill out the following forms.
Informed Consent
Each of our Clinical Associates is a licensed practitioner. As required by licensure, treatment under a licensed practitioner is confidential. This form explains limitations to that confidentiality required by licensure. It also outlines the collaborative nature of the therapeutic relationship and co-responsibility for treatment outcomes.
Financial Agreement
This form outlines The Viva Center’s financial policies. Here at Viva, we offer special accommodations to assist with cash flow and simplify payments. As a professional courtesy, we do not bill clients opting to pay with a credit card until the end of the month, although technically, fees are due at the time of service. This allows time for insurance to send reimbursement closer to the time of payment.
Client Questionnaire
This form serves 2 purposes:
- helps to capture the baseline experience when beginning treatment, allowing for an opportunity to take stock of current status
- helps the clinician gather important background information without taking extra time in the first session.
The first page of this form is required, and the others are supplemental. It is fine to exercise the decision to leave out any areas of the form. Please keep in mind any areas elected not to be disclosed at this time may be relevant to treatment outcomes.
Credit Card Authorization Form
This form is only required if electing to use a credit card to pay for services. Conveniently, you only need to fill out this form once, and we securely store it. We also accept payment with cash or checks.
